BREAKTHROUGH! is specifically designed to meet the unique needs of 18 to 24-year-olds, the fastest growing homeless population on the coast! Many of these vulnerable young adults are “aging out” of foster care and may have lived in multiple households and attended numerous schools and are particularly susceptible to human trafficking, addiction and homelessness.

Community Care Network, in collaboration with youth courts and other partners now assists these young people by providing housing, Life Skills training, mentoring and counseling designed to address their unique needs and to encourage them to become self-reliant as they BREAKTHROUGH to a new future.
